Annual Health and Medical Record

  • For Scouts and adults spending more than 72 hours at camp a yearly physical form signed by a licensed practitioner must be complete prior to coming to camp.  For less than 72 hours, personal and medical history and signature of parent (if scout is under 18) is the least requirement.
