MEDICAL FORMS
Class 1 and 2

Needed for all scouts; parents must re-initial and re-date every 6 months; doctor's signature is good for 3 years

Class 1 section required for adults staying less than 72 hours (no doctor's signature required); Class 2 required for adults under 40 staying more than 72 hours with doctors signature from within last 3 years

Required for High Adventure participants; parents must re-initial and re-date every 6 months; doctor's signature good for 1 year

Annual Health and Medical Record
- This is the new Medical Record that will be required beginning January 2010. You can use this if you wish, it is only good for 12 months and has a 72 hour period or longer section much like the old forms.
